Sinai Mile Square
Community Mental Health Center
2040 W. Washington Blvd.
Chicago, IL 60612
(312) 850-5800

Sinai Mile Square Community Mental Health Center, part of Mount Sinai Hospital's Department of Psychiatry and Behavioral Health, provides outpatient services such as mental health assessment, treatment, case management, and psychiatric evaluation for children, adolescents and adults. Specialized services include Screening, Assessment, and Supportive Service (SASS) for children and adolescents, and Linkage Case Management for adults.

The center provides culturally sensitive treatment that builds upon the resilience and strengths of individuals and families. The primary service area is the city's Near West Side.

Clients may be self-referrals or referred by others, and may come to the center as walk-ins, scheduled arrivals, or through telephone contact.

Referral Process

  • Call (312) 850-5800 to schedule an intake appointment. The center is open for walk-ins from 8:30 a.m. to 5 p.m., Monday through Friday.
  • Individuals requiring emergency care (hospitalization) may be referred to Mount Sinai Hospital's Emergency Department or Madden Mental Health Center.
  • Individuals requiring crisis care (crisis residential assistance, immediate treatment, and/or access to food, clothing or shelter) are seen immediately for an initial screening, assessment and disposition.
  • Individuals not requiring emergency or crisis care are referred to other services (external or internal) based upon presenting symptoms and history.
  • Following assessment, individuals work with a multi-disciplinary staff to develop a treatment plan that identifies specific goals and objectives.

Adult Services

Individual, group and family therapy; crisis intervention; psychological and psychiatric evaluation are offered. Groups for individuals with a dual diagnosis (MI/SA) also are available.

Youth and Family Services

Services include individual, group and family therapy; crisis intervention; psychological and psychiatric evaluation; case management; and a therapeutic summer day camp.

SASS

Screening, Assessment and Supportive Services provides 24/7 screening and crisis intervention services to seriously emotionally disturbed children and adolescents. The program provides treatment in the least restrictive environment, linking clients to appropriate outpatient or inpatient psychiatric services, monitoring psychiatric hospitalizations, and ensuring that they are linked to community services on discharge.

The program works to shorten hospital stays and avoid future hospitalizations. It also may link families with alternatives to psychiatric hospitalization, such as intensive in-home services that include individual and family counseling and respite care.

Case Management

Adult clients get experienced help securing basic necessities, such as food, in-home support, housing, medical and dental care, and other social, educational, vocational and recreational services.

Linkage Case Management

This is intensive case management targeting adults deflected or discharged from psychiatric inpatient care who have difficulty following through with outpatient care. The goal is to ensure successful linkage to outpatient behavioral-health care.

Sinai Mile Square CMHC is operated by Mount Sinai Hospital's Department of Psychiatry and Behavioral Health. Programs are funded by a grant from the Illinois Department of Human Services, Division of Mental Health.
