Home Health

Mount Sinai Home Health provides care to patients who find it difficult to - or can't - leave their homes. Our Home Health team includes:

  • Nurses
  • Certified wound care nurses
  • Home health aides
  • Physical therapists
  • Occupational therapists
  • Speech therapists
  • Social workers

Home care services are covered by Medicare, Medicaid and many health plans.

Mount Sinai Home Health is a Medicare-certified home health agency, licensed by the Illinois Department of Public Health and accredited by the Joint Commission on Accreditation of Healthcare Organizations.

Mount Sinai Home Health monitors patients' health and provides services that include:

  • Wound care
  • Adult & Pediatric IV therapy
  • Monitoring of medications
  • Teaching family members to care for the patient at home
  • Help with bathing and other personal care
  • Evaluation of home safety
  • Discussion of diet and nutrition
  • Therapy to develop walking skills
  • Teaching mobility techniques for those who cannot walk
  • Speech therapy
  • Follow up on rehab programs begun in a hospital or other facility
