Making Tracks to Transition
Description of program: What is Making Tracks to Transitions?
Making Tracks to Transitions is a program that provides culturally competent, family-centered care, aimed
at providing assistance through the transition to adulthood. The program ultimately helps empower youth
with disabilities to successfully reach their highest levels of independence. It does this by teaching
appropriate life skills and by providing participants (ages 14-22) and their families with information on
appropriate health care, resources and adult service programs.
Medical
The Making Tracks to Transition program at Schwab provides support to participants and their families by
assuring that their medical and wellness needs are met. We meet their needs by providing individual case
management and a clinic devoted to transitional issues. We assist participants in the transition from
their pediatrician to an adult primary care provider and help them to maintain good health by teaching
them basic self-care skills.
Recreation
The Making Tracks to Transition program at Schwab addresses psycho-social issues by providing adolescents
with a network of peer mentors with similar disabilities and encouraging participation in recreational
activities. Through these experiences youth gain the skills and confidence necessary to develop meaningful
social relationships and fully participate in their communities.
Independent Living
The Making Tracks to Transition program at Schwab also provides participants and their families with
information on learning skills in the home, resources related to accessible and supportive housing, as
well as potential educational and employment opportunities in order to help each individual reach their
maximum level of independence.
