Please contact our admitting department at 773-522-5895 if you would like to make a referral to Schwab. You can also contact our admitting department if you, a family member, or friend are currently an inpatient at another facility seeking to transfer to Schwab Rehabilitation Hospital for inpatient rehabilitation and/or specialized medical services.
