Admission and Referral Information
Inpatient Information
Select "Inpatient Information" If you would like to make a referral to Schwab. You can also select "Inpatient Information" if you, a family member, or friend are currently an inpatient at another facility seeking to transfer to Schwab Rehabilitation Hospital for inpatient rehabilitation and/or specialized medical services.
Outpatient Information
Select "Outpatient Information" if you would like information about outpatient services or are interested in scheduling an appointment.
